Mostly based on mythology
Skeletons have many myths from all around the world such as the Mexican sugar skulls, but most depictions of skeletons in d&d are based off of the grim reaper from the middle ages. Its described as a skeleton draped in tattered black and blue robes holding a scythe and or an hourglass. In d&d, skeletons are long decayed corpses risen from the dead by necromantic magic, usually wearing armor scraps and carrying a short-bow and short-sword.
